Please fill out the online form provided below to register your dog for obedience classes. Obedience classes run one night per week for eight weeks. Classes with four or less students may be shortened. For any questions regarding the application process, please contact the registrar. **These classes are not suitable for aggressive or reactive dogs, which will be dismissed.

Class fee is $226.00 (including HST). Please send e-transfer of fees to sdkcregistrar2015@gmail.com following completion of this form. REGISTRATION IS NOT COMPLETE UNTIL FEES ARE PAID.

Agreement to Enroll in a Sudbury and District Kennel Club (SDKC) Class. Check all if you agree:*

1. In consideration of my enrollment and participation in the SDKC classes, I hereby agree to assume all liability for any personal injury including communicable diseases, property damage and injury, or damage to other dogs, people and the staff of the SDKC which may be caused by me or the dog(s) I bring to class. I further agree to indemnify and hold harmless the SDKC instructors and staff and sponsors from any such liability.

2. I understand that the instructor may dismiss me and my dog from attending any classes if he/she feels my dog is a threat to another dog, another student or the instructor(s). Any dogs without proof of current vaccinations will be considered a threat to other dogs in the class. Any Dismissal from attending class is at the full discretion of the instructor(s). If dismissed from attending classes there will be no refund of costs.

3. I understand that I will be dismissed from the class if I fail to clean up after my dog either inside or outside the training centre. Use of either prong or e-collars in class will also result in dismissal.

Reactive dogs must be identified to the registrar before payment and may be asked to wear a muzzle at the discretion of the instructor. Failure to comply will result in dismissal. Further disruptive behavior in spite of muzzle use may result in dismissal.

5. I understand that bitches in season cannot attend class. Handlers can attend and practice the lessons at home with the dog.

6. I have read and will abide by the above enrollment agreement on this form. I understand that a $10 handling fee will be applied to any fees refunded for class payments.

I authorize the release and agreement for SDKC to use pictures of video of me and my dog(s) on the SDKC website. (If under 18, please obtain permission from a parent or guardian first.)*
